What information do you need to assess my benefit claim
The following information is need to asses your benefit claim:
A completed benefit form
Two original documents for proof of identification for both yourself and your partner (if you have one)
An original document showing a National Insurance Number for yourself and your partner (if you have one)
Proof of income for yourself, your partner (if you have one) and anybody else that lives in your household
Proof of any savings or capital you or your partner have
